Day Training & Habilitation (DTH)
The DTH programs at Opportunity Partners offer people with disabilities opportunities to learn new skills, gain greater independence, explore the community and develop friendships. Services are customized, based on individual needs and goals. Whenever possible, activities center on community involvement in order to offer a wide variety of experiences and opportunities.
Opportunity Partners’ employment and community services programs have earned three-year accreditation – the highest level available – from the national Commission on Accreditation of Rehabilitation Facilities (CARF).
Employment
Employment options are designed based on the skills, job interests and goals of each individual. Community employment opportunities range from Supported Employment Teams (SETs), which include an on-site supervisor, to independent employment with the support of a job coach. Opportunity Partners also offers center-based employment options, providing individuals the chance to experience a variety of jobs in a more supportive setting.
Education
Life-long learning is an important component of Opportunity Partners’ DTH programs. Individuals can supplement their employment activities with a wide range of classes relating to career development, health and safety, and increased independence. Classes are based on the interests and goals of individuals, and encourage community involvement.
Person-Centered Planning
Opportunity Partners uses a person-centered assessment to identify the individual strengths, interests and goals of program participants. Annual team meetings involving the individual, family members, staff, social workers, therapists and other professionals are designed to mark progress, identify new outcomes, and encourage continued growth.
Opportunity Partners’ DTH programs are based out of service locations in Minnetonka, Plymouth and Coon Rapids. All programs include transportation supports and coordination with residential and other providers. Speech, physical and occupational therapies are available through sub-contracted services.
