Semi-Independent Living Services (SILS) is an option that provides 1-13 hours of service per week, depending on need. The individual must be able to financially support their housing expenses from earned and unearned income and must be able to demonstrate self-advocacy and survival skills.
In-Home Waiver (IHW) provides up to 24-hour support from an off-site location. Individuals need to be able to make informed choices and be able to financially support their housing expenses from earned and unearned income. In addition, the individual must be eligible for medical assistance or be a current recipient.
Independent Living Program (ILP) is an option available in Hennepin County that provides about four hours of on-site support per day, with extra support on weekends, and has a 24-hour support component from an off-site location. Individuals must be able to make informed choices and be able to financially support their housing expenses from earned and unearned income.
Establishing a Firm Foundation
As people move out on their own, we can ease the transition. We've created an extended partnership with landlords to promote safety and accessibility in housing. Every effort is taken to ensure that affordability, location and convenience are primary considerations in the selection of a residence. We also help individuals screen roommates for compatibility. This includes introductions, interviews and a careful analysis of common interests and other compatibility factors. We believe a good roommate contributes greatly to a person's quality of life and potential for success in community living.
Supporting Independence
Once the transition is completed, we continue to provide assistance. We can help plan meals, manage budgets, teach first aid and medication usage, develop personal grooming skills, and provide education on public transportation and other community resources. We work closely with the people we serve, their families and referral sources to develop services that meet individual needs and schedules.
