Youth Transition Services
Opportunity Partners’ Youth Transition Services assist families and students (ages 14-25) as they successfully transition from academic learning to on-the-job education. Students with disabilities explore careers and gain hands-on work experience. Our Employment Specialists work closely with students, parents, educators, vocational rehabilitation counselors and others, offering the students choices and support to reinforce decision-making skills and independence. The program is designed to ensure a smooth transition for youth with disabilities into adult services.
Youth Transition Services include: Vocational Evaluation, Supported Employment Teams, Job Coaching and Job Development. Through these support services, we provide the right environment for students to develop positive work skills while exploring their interests.
We offer the following options:
School to Work Contracted Services - Opportunity Partners contracts with a number of school districts to provide students with disabilities experiences, including employment and community service, that the schools are not equipped to provide. Students may access for a quarter, semester, school year or multiple years of service.
Career Camp - Opportunity Partners’ Career Camp helps youth with all kinds of abilities set reachable dreams and goals for careers, personal well-being and growth. A five-day experiential opportunity for transition-age high school students, this camp is designed with fun in mind. Campers learn through games, employer tours, and activities to explore the world of employment and reachable career goals. Opportunity Partners also offers a Career Camp Sampler Day to give students and parents a chance to check out the program.
Career Camp on the Road - Let us come to you! We'll pack our car with classroom activities to help teachers in the classroom, parents on retreat, or conference attendees to develop the skills of young adults with disabilities. While the adults are learning, our qualified employment specialists will also instruct students using a variety of games and activities.
Navigators - This exciting series is a collaboration of more than 20 community partners who gather to empower youth and parents who are managing the challenges of transition. Partners have included Adaptive
Park & Recreation for the Cities of Bloomington, Richfield, Eden Prairie and Edina, the Metropolitan Center for Independent Living, City Police Departments, PACER Center, and more.
