Most individuals at this location are involved with our day training and habilitation (DTH) program. Individuals are employed within the community but some work at our on-site production facility. This location is also the site of our School to Work program. Opportunity Partners' corporate offices are also housed at the Minnetonka facility.

This location serves over 100 people and provides day training and habilitation (DTH) services which promote independence, including employment in the community and at our on-site production facility.
Golden Opportunity
7100 Shady Oak Road, Eden Prairie
This location offers day services to seniors with developmental disabilities. Options include employment, volunteerism, community activities, classes and more in a supportive environment.
Karlins Center 5480 Nathan Lane, Plymouth This location offers one of Minnesota's only day training and habilitation service specializing in promoting independence for adults with autism or severe behavioral needs. It features a wide range of social, educational, recreational and employment opportunities both within the community and on-site.
TBI Metro Services
8 West 66th Street, Richfield
20 East Thompson Avenue, Suite 101, West St. Paul
These sites offers community integration, job training, and job placement services for adult survivors of brain injury. The Community Integration Service (CIS) focuses on real-life experiences in the community by teaching communication techniques, stress management, and compensatory skills. Volunteering to obtain job experience is an important part of the CIS program. An Independent Living Skills (ILS) program also offers in-home services. Advance Service is a bulk-mail, packaging and assembly operation that focuses on enhancing vocational skills through meaningful work experience. Job development staff are also available to assist people ready for competitive employment opportunities.
Opportunity Partners - Anoka
3351 Coon Rapids Blvd., Coon Rapids
The programs at this location offer Semi-Independent Living Services (SILS), In-Home Waiver Services (IHW) and Day Training and Habilitation Services (DTH) to residents of Anoka County. The SILS and IHW programs serve nearly 100 people by providing the services people need to live on their own. The DTH program features a wide range of employment and personal planning services, as well as job placement assistance.

These offices provide Vocational Rehabilitation Services, including independent job placement, skill evaluations, and support services that help clients grow in their jobs. We work with the clients to meet individual interests and skills, and hlp ensure long term employment success.
Opportunity Partners - Lake Harriet

Opportunity Partners' Welfare to Work service provides supports to individuals on the MFIP Tier II system. The goal of services is to provide guidance and support for people to secure meaningful employment that generates more than enough money for self-sufficiency.
Opportunity Partners - Nicollet
3420 Nicollet Avenue South, Minneapolis
This office houses the Hennepin County Semi-Independent Living Services (SILS) program, In-Home Waiver Services (IHW) and Independent Living Program (ILP). These programs serve nearly 100 people by providing the skills individuals need to live on their own. Assistance is provided in everything from planning meals and managing budgets to assisting with health care needs.
Locations of Community Homes
Opportunity Partners operates 20 homes in the Twin City metro area. Adults with developmental disabilities are served in these homes that feature 24-hour supervision by Opportunity Partners' staff.
